Based on record review, the center did not ensure at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members obtain and submit the results of a follow-up tuberculosis screening. Evidence: The record of staff #6 (start date: 6/22/17) had documentation of a TB screening dated 6/15/17. Staff must submit documentation of a follow-up tuberculosis screening at least every two years.
