Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 9/27/2021 and concluded on 9/28/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 76 children present, ranging in ages from three months old to four years old, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review and discussion with staff, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. The documented first day of attendance for Child #4 was 4/26/2021. The school entrance form with documented immunizations was not acquired until 4/30/2021.

Description:
Based on record review and discussion with staff, the center failed to obtain a copy of the results of a criminal history record information check and a sex offender registry check from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #3's documented date of hire was 4/7/2021. The record for Staff #3 did not contain an out-of-state search of the sex offender registry check nor did the record contain an out-of-state criminal history check.
