Comments:
An unannounced monitoring inspection was conducted on 8/10/2023 from 9:53 am to 12:45 pm. There were 43 children in care, ages two months to five years, in six groups under the care of eleven staff. The Licensing Inspector reviewed three children?s records, two medications and associated records, three staff records, and other program records. The children were observed in classrooms during indoor play and washing hands.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
The record for child 3 does not have updated immunization documentation, which is required every six months for children under the age of two years.
EVIDENCE: The most recent immunization records for Child 3, 17 months old, were dated 6/21/2022.

Based on record review, the center failed to have some required information in a child record.
EVIDENCE: 1. The record for child 1 did not have a doctor name and phone number.
2. The record for child 3 did not have documentation of child updates and confirmation of up-to-date information as required by 8VAC20-780-420-E-3, which is required at least annually. The most recent information in the record is dated 6/7/2022.
Plan of Correction:
Child 1 was registered for the center before birth, and the doctor information was not yet available. From now on, registration information will be reviewed for completeness when a child starts attending.
The parents will be asked to review registration information, make updates as needed, and sign off on it.
