Comments:
An unannounced monitoring inspection was conducted on 09/26/2023 from 11:20am to 1:15pm. There were 59 children ages 3 months to age 4 years old supervised by 13 staff. The physical plant, programming, attendance, menus, 5 staff records, 5 children?s records, 2 medications with corresponding authorization records, emergency drills, bus, and emergency supplies were reviewed. The playground was not inspected due to inclement weather. Children were observed participating in nap, lunch, and television time. Diapering and handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on review of 5 staff records, the center did not obtain the results of a criminal history record check from all states in which staff members have resided within the last five years prior to employment.
Evidence: The record for Staff #2 (date of hire 9/19/23) did not contain documentation of a completed criminal history record check from all states where they resided within the last five years prior to employment.
Plan of Correction:
It is our understanding that we are required to receive a potential employees criminal background check and TB test prior to starting. This was completed before the employees start date. It is to our understanding that all of our state clearances are to be completed within 30 days after the staffs start date. It has now become clear that criminal background checks for states that do not participate in the NFF state wide background checks where staff has previously resided in these states within the last 5 years. We are now made aware that it has to be done before the employee?s start date. The current violation in which we received has already been initiated and we are waiting to hear a response from the local agency for these results.
