Comments:
An unannounced monitoring inspection was conducted today from 10:14am-10:50am with the provider on site. There were 6 children (12 points) with 2 caregivers. A sample of 2 children's files and 3 caregivers/household member's files were reviewed. The children were engaged in the following: playing with play dough, singing and playing with toys. Interactions between the children and caregivers were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, attendance records, first aid kit, radio, flashlight, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. The Provider does not have Medication Administration Training. Areas of non-compliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on review of household records, the family day home did not obtain the Central Registry Results every five years for one household member. Evidence: 1. Household member #2 had a central registry result dated 10/02/2018 and had not been updated every five years.
