Technical Assistance:
Technical assistance was offered regarding the following: hand washing, children's records, transportation.
Comments:
An unannounced monitoring inspection was conducted from 4:15pm through 5:25pm. There were 5 children (3 points) with one caregiver. The children were observed playing on a community playground, eating snack upon arriving to the family day home and getting picked up. Fire drill records and 5 children's records were reviewed. Based on record review, the provider did not obtain documentation of a physical exam within 30 days of enrollment. Evidence: Child # 4's first day of attendance was 8/28/18. Documentation of a physical exam was not obtained for Child # 4 within 30 days of enrollment.
Plan of Correction:
I will ask the parents to bring in the physical.

Description:
Based on records review, the provider did not maintain complete records for children enrolled. Evidence: 1. Child # 1, Child # 2 and Child # 3 were missing the annual update. The last update was done in August 2018. 2. Child # 2 and Child # 3 were missing information for persons authorized to pick up.
Plan of Correction:
I will get all the missing information from the parents.
