54.1 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Technical Assistance was offered regarding: background checks for household members reaching the age of 14 years; play equipment used for children; menu, specifically related to food items that could pose a choking hazard.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 2/22/21. The provider was contacted by telephone to initiate the inspection. There were 6 children (8 points) and 1 caregiver present. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver's records virtually to ensure documentation was complete. Emergency drill records, medication administration certification and the emergency preparedness plan were reviewed. A virtual walk through of the family day home was also conducted. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination for a child within 30 days after the first day of attendance.
Evidence: Child # 1's first day of enrollment was 6/22/20. The physical exam report was not obtained by the provider within 30 days of enrollment, and was not available for review.

Plan of Correction: I have requested the report from the parents.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of immunizations for children prior to the first day of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 6/22/20.
2. Child # 1's immunization records were not obtained, and were not available for review during the inspection.

Plan of Correction: I have requested the immunization records from the parents.

Standard #: 22VAC40-111-500-F
Description: Based on observation, trampolines were being used by children in care.
Evidence:
1. During the virtual inspection, it was observed that there were two indoor small trampolines in the basement of the family day home.
2. One child was observed using the trampoline.

Plan of Correction: The trampolines will be removed and not used again for daycare children.
