Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, November 21, 2019 from approximately 12:30 PM until 3:00 PM. During the inspection, the licensing inspector observed the following ratios: Infants: 4 children with 2 staff Ones and Twos: 5 children with 2 staff Threes and Fours: 7 children with 1 staff School age children were not present during the inspection.
The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing with musical instruments, looking at books, going down the indoor slide, learning about Native Americans and Pilgrims while making Thanksgiving crafts, taking bathroom breaks and washing hands, eating lunch and resting at nap time. All areas of the center including classrooms, hallways, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: daily attendance, parent agreements, staff training information, required postings, emergency drills and insurance policy. The center?s first aid kit and emergency supplies were inspected. The center provides transportation and the van and transportation supplies and documents were reviewed. Members of management stated medication is not administered at this time. Five children and five staff records were reviewed

Based on review of five children?s records, the center did not ensure documentation of a physical exam was obtained for one child.
Evidence: The record for Child #5 (start date 5/1/19) did not contain documentation of a physical exam.
Plan of Correction:
The parent was contacted during the inspection and asked to bring a copy of the child's physical to place in the record.

Description:
Based on review of five children?s records, the center did not ensure all required information was in one child?s record.
Evidence: The record for Child #1 (start date 11/11/19) contained the name, address and phone number for one emergency contact.
Plan of Correction:
Information was obtained and added to the record while the licensing inspector was on site.

Description:
Based on review of five records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff member prior to employment. Evidence: 1. The record for Staff #2 (hire date 10/15/2019) did not contain satisfactory results of the finger-print based national criminal background check. 2. Staff #2 was observed working alone with the children during the inspection. 3. Staff #2 stated she went to Virginia State Police and completed the fingerprinting process. 4. Fingerprint-based national criminal background checks obtained after 01/21/2018 are required to be completed through the department?s vendor, Fieldprint, so the results come directly from the Virginia Department of Social Services.
Plan of Correction:
An appointment to complete the background check was made while the licensing inspector was on site.
