Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 06/12/2020. The director was contacted by telephone to initiate the inspection. There were 0 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the center did not ensure that one of two children's records had documentation of immunizations prior to enrollment.
Evidence: 1. Child #1 (DOE: 09/04/2018) had an immunization record dated 09/19/2018. 2. Administration confirmed that the immunization records were received after enrollment.
Plan of Correction:
We will schedule ourselves to review student immunization and physical expirations monthly to ensure the files stay up to date.
