Technical Assistance:
Discussed with administration: Annual update training, allergy action forms. Owner stated during the inspection that they intend to contact Subsidy and cancel their vendor agreement. They no longer wish to be a Subsidy provider.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on 03/11/21 and concluded on 03/12/21. The in-charge person was contacted by telephone to initiate the inspection. There were 49 children present and 9 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.
Based on a review of records, children's records do not include all required information. Evidence: 2 of 3 records reviewed for children with diagnosed food allergies, do not include an allergy action plan.(Child #1, Child #2)

I will ask the parents to have the doctor complete an allergy action plan for each child.

Based on a review of records and administration staff statement, staff have not completed the required annual training. Evidence: No staff have completed the required annual update training (3 hour course).
Plan of Correction:
Staff will be required to complete the 3 hours of training within seven days, once we receive the information for the course.
