Comments:
A Subsidy Health and Safety inspection was conducted on 2/11/20 from 3:00pm - 4:30pm. During the inspection there were 57 school age children in care with five staff. Children were observed participating in various activities, eating snack, and doing homework. Records were reviewed for six children while at the facility. The records for five staff were reviewed at the central office on 2/12/20. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Based on a review of five staff records, it was determined that the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence: 1. The record for staff #4 (date of hire 1/15/19) contained documentation of a TB screening that was dated 1/17/19. 2. Staff #6 confirmed that staff #4 had not completed a TB screening at the time of employment.
Plan of Correction:
The vendor responded: Going forward we will ensure that all new staff had completed a TB by the time of employment, and that it has been completed within the last 30 days prior to employment.
