Comments:
A monitoring inspection was initiated on 2/15/2023 and concluded on 2/15/2023. There were a total of 12 children ( 24 points) including 2 of providers own children, observed with 2 caregivers present. The children were engaged in the following: morning snack, diapering, handwashing and play dough. The home, 3 child records, 3 caregiver/household member records, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of records and provider interview, the provider has not maintained documentation of the current rabies vaccination. Evidence: One pets rabies vaccination expired 1/2023 and has not been renewed.
Plan of Correction:
Appointment for rabies is on 2/22/2023.
