Comments:
The inspector conducted an unannounced mandated inspection from 1:45 until 3:30 p.m. There were 11 children in care with two staff members present. No medications are being administered. Children were observed playing inside and learning letters, playing outside, napping, and being picked up by their parents. Five children's records and five staff records were reviewed. Please contact your inspector at 276-698-9981 with any questions.
Based on record review, the center did not have all the required information for new staff.

Based on record review and interview, the center did not complete an out of state central registry check for a person that had lived out of state in the past 5 years.

1. The inspector reviewed five staff records. One of five records (#1) had completed the sworn disclosure stating they had lived out of state, but there was not out of state central registry check completed.
Plan of Correction:
The director will have the out of state check completed.
