Based on documentation from a local agency and photograph, the family day home failed to be maintained in a clean and safe condition.
1. On October 30, 2020, local agency observed black mold on the ceilings of the bathrooms in the family day home. One of the bathrooms is used for the children in care. 2. The provider confirmed there was black mold on the both bathroom ceilings.
Plan of Correction:
ServPro completed mold remediation and cleaned and restored the areas that were affected. Contract and certification was given to the licensing inspector.
