Comments:
A monitoring inspection was conducted on 11/17/2023 between the hours of approximately 9:45 am and 10:45 am. There were 12 children present, ranging in ages from 16 months and 4 years, with 2 caregivers supervising. Children observed while working in decorating pumpkins, toileting, reading books, and eating snack. The morning snack today consisted on pop tarts, apple juice, and banana slices. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 5 staff/household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, Central Registry search (CRS) results were not obtained by the end of the 30th day of employment for one caregiver Evidence: 1) On the date of inspection, the record for Caregiver #4 did not contain CRS results that had been requested by the provider. A copy of CRS results was available in the record but they were from a previous employer.

Description:
Based on observation and interview with the provider, medications for children in care were not stored separately from medications for household members and caregivers. Evidence: 1) On the day of inspection, a bottle of Motrin pills was found among the children's medications. The provider stated that the pills were hers.

Corrected during inspection. The provider removed the bottle of pills.

Description:
Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) On the date of inspection, children's medications were located in a drawer under the changing table. The drawer was not locked.

Based on record review, the provider did not ensure that caregivers and household members obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years. Evidence: 1) The Tb screening form in the record for Caregiver #2 was dated 03/24/2021.

Description:
Based on observation, the diapering surface did not meet all requirements. Evidence: 1) The diapering surface shall be nonabsorbent and washable - On the date of inspection, the diapering mat had a tear, making it not-nonabsorbent.
