Comments:
The licensing inspector conducted an unannounced focused monitoring inspection on Tuesday, September 3, 2019 between 11:40 am to 12:40 pm. During the inspection, there was observation of one children present totaling three points and one caregiver at the family day home. The child in care was watching television, interacting with provider, engaged in active play, and eating lunch. The lunch for today consist of American cheese, bread, mixed vegetables, pears, and milk. The licensing inspector reviewed the designated children care areas, bathroom, and kitchen area in the family home to determine compliance with licensing standards. There were six children's, one caregiver's, and one household member's records reviewed during this inspection. Based on review of children's record and interview, the provider did not ensure to obtain a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: In Child #3's and Child#5's records (start date: 05/17/19) there was no documentation of a physical found. Interview with provider confirmed that a physical examination for both children have not been received at this time.

The provider will make contact with parent today to request documents.
