Arrived 10:25am Departed 11:48am 10/21/22 An unannounced Code compliance inspection was conducted today. There were 13 children in care with five staff members. The children were coloring, playing with toys, eating lunch and resting. Two child records and two staff records were reviewed. Thank you for your cooperation. Based on a tour of the building and interviews with staff #1 & 2, the facility failed to ensure that a staff member is regularly present with each group of children. Evidence: 1. Child #2 (age 6 months) was sleeping alone in a classroom with the door closed.

Based on observation, the facility failed to ensure that all Safe Sleep Practices issued by the American of Academy of Pediatrics are followed. Evidence: 1. There were bumper pads in the crib occupied by child #2. 2. Child #1 (age 3 months) & 3 (age 2 months) were sleeping in swings.
