Based on record review and interview, the provider did not obtain a subsequent background check every five years for each adult living in the family day home. Evidence: Household member #1s most recent central registry background check is dated 2/20/2019. The provider confirmed that a repeat central registry background check has not yet been completed.

I didn't realize that 5 years have passed. I will have it done right away.
