There were 23 children under the care of 2 staff. The ages of the children ranged from five to ten years old. The children were observed during outside time and indoor play. Five children?s record, and three staff records were reviewed. The inspector also took measurements on classrooms that had been added. 3. On the back area of the playground there was a broken metal gate laying on the ground. This is and entanglement hazard for children.
4. In the mulched playground area where children were observed playing, there was a metal rod on the ground. The rod was about 4 feet long. This is an impalement hazard for children.
