Comments:
An unannounced focused inspection was conducted on November 14, 2019. Areas of focus were administration, physical plant, special care provisions and emergencies, background checks and staffing and supervision. Areas of noncompliance are identified on the Violation Notice. The exit interview was conducted with the Owner.
Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence: The owner confirmed that the chain was broken and detached from the outdoor playground equipment used to access the stairs to the top of the structure.
Plan of Correction:
The playground equipment will be repaired. The children will not be allowed to play on that piece of equipment until it's been repaired.

Description:
Based on record review, in one of six staff records reviewed, the licensee did not ensure that staff must be denied continued employment if the center does not have a central registry finding within 30 days of employment.
Evidence: The owner confirmed that there was not a Central Registry finding for Staff #3 (date of hire 9/9/19).
Plan of Correction:
Owner contacted the Office of Background Investigations because the employee did not know the last name and therefore we could not process the form until he provided the last name. The staff member informed us yesterday of the last name.
