Comments:
A Subsidy Health and Safety inspection was conducted on 11/18/2019 from 9:35am to 1:45pm to review supplemental health and safety requirements. The children were observed learning through play in centers, singing, participating in story time, coloring and eating morning snack. Morning snack consisted of crackers, cheese and water. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today.
Five children?s records and four employee records were reviewed during this inspection. Five injury reports were reviewed. The center administers medication; however they did not have any being given at this time.
The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/15/2019 Last shelter-in-place drill: 10/15/2019 Last fire inspection: 07/11/2019 Last health inspection: 11/18/2019
Today, the following child to staff ratios were observed: Infants, Toddlers, Twos ? 3:1 Threes and Fours - 8:1 School Age - 4:1
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A Violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Based on record review, the center did not ensure that one of four staff records had documentation of TB results completed within the last 30 calendar days of the date of employment. Evidence: The record of staff #3 (start date: 9/10/19) had documentation of TB results dated 4/15/19.
Plan of Correction:
The staff will get a new TB test today.
