Comments:
An unannounced renewal inspection was conducted on October 2 2019 between 10:15am and 12:30pm. There were 3 children in the direct care of two staff members. Children were observed learning colors, completing puzzles, washing hands, and playing with various toys. The center is clean and organized. It offers age-appropriate materials that the children can utilize. Areas of non-compliance are listed on the violation notice. Based on record review, 1 of 5 children's records reviewed did not contain documentation that the child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #4 record did not contain documents of immunizations the child has received.

Description:
Based on record review, 1 of 2 staffing records reviewed did not contain documentation of a sworn statement or affirmation.
Evidence: The director stated that the employee began working the week of 9/22/19 - 09/27/2019. The staffing record does not have a completed copy of the sworn statement form.

We will make her sign the form today.
