An unannounced monitoring inspection was conducted on June 23 2023 between 10:00am and 11:30am. There were 6 children in the direct care of 1 staff member. A review of children's records, staffing records, the physical space, evacuation drills, and emergency supplies was conducted. Citations are listed on the violation notice. Based on record review, the center did not ensure a repeat tuberculosis screening was submitted every two years from the first initial screening.
