Technical Assistance:
Discussed the importance of food storage to ensure that food remains fresh and sanitary.

Provided the Intrastate Background resource to request out of state background checks for staff.

Comments:
An unannounced Monitoring inspection was conducted this date. A total of 52 children with 11 staff were onsite. The children were transitioning to nap time. Staff were observed assisting with setting up mats and linens for the children. They were observed sitting beside the children and providing adequate supervision. All classrooms were observed today. One infant sleeping in a crib while the rest of the children in attendance in the infant class was sleeping on mats. Older children were engaged in quiet activities in the large multipurpose room along with other children napping. Four new staff records were reviewed along with 4 children's records. 2 children with medication needs. Medication and authorizations were also reviewed. Description: Based on staff record review, one background check was not obtained within 30 days of hire.
Evidence:
Staff #3 lived out of state during the past five years. A Central Registry search result has not been obtained.

Plan of Correction: An out of state background check will be obtained for this staff and forwarded to the Licensing Inspector for review within 10 days.

Standard #: 8VAC20-780-240-A
Description: Based on staff record review, one staff did not complete required orientation training.
Evidence:
Staff #1 was hired on 10/22/2021. No documentation that Staff #1 completed the 10 hour VDOE sponsored orientation course.

Plan of Correction: This staff will complete the 10 hour preservice training in two weeks.

Standard #: 8VAC20-780-520-A
Description: Based on review of topical ointments, one ointment was kept beyond the expiration date of the product.
Evidence:
One topical ointment was found onsite with an expiration date of 2020.

Plan of Correction: This topical ointment will either be returned to the parent or disposed of.

Standard #: 8VAC20-780-520-C
Description: Based on observation of the center, topical ointments were found accessible to children in care.
Evidence:
One topical ointment was found within reach of children in care in the hall bathroom located on the left side of the school.

Plan of Correction: The topical ointment was relocated to an area not within reach of children.

Description: Based on review of the emergency evacuation log, monthly evacuation drills were not completed.
Evidence:
A monthly evacuation drill was not completed in April 2022.

Plan of Correction: We will ensure that a monthly fire drill is conducted even if a shelter in place drill is conducted the same month.

Description: Based on review of shelter in place procedures, a shelter in place drill was not completed twice a year.
Evidence:
Only one documented shelter in place drill was conducted during the past year.

Plan of Correction: A shelter in place drill will be conducted before the end of December. Two drills will be completed and documented each year.

Standard #: 8VAC20-780-560-N
Description: Based on observation, food was not stored or prepared in a sanitary manner.
Afternoon snack occurs after nap time. Children's snack food which consisted of yogurt, fresh fruit and fresh vegetables and string cheese was left open and unattended on tables for up to 2 hours while the children nap.

Plan of Correction: We will leave food covered or wrapped or refrigerated while the children nap.
