Technical Assistance:
Discussed the supervision point system with the Provider.
Comments:
An unannounced renewal inspection was completed today. The physical plant, 3 caregiver/household member files, 3 children's files, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 10:40-11:50am. There were 3 children, totaling 11 points, under the supervision of the Provider. Today, the children played with puzzles, listened to storytime, enjoyed free play, ate lunch and rested during nap time. The interactions between the children and caregiver was seen to be positive and constructive in nature. Today, the children were served chicken with rice, mixed vegetables, pears and milk. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs at this time. 1. Child #1 is 20 months old. The most recent immunization record on file was completed when the child was 12 months old.

Description:
Based on observations, the bathroom that the children use does not have all required items in it.

1. The bathroom did not have paper towels in the bathroom.

Description:
Based on observations, the diapering surface was not non-absorbent.
Evidence
1. The diaper mat had a few rips in it, which prevented it from being non-absorbent.
Plan of Correction:
The Provider will replace this.
