Comments:
Licensing Inspector arrived at 10:15 and departed at 11:30 AM to conduct a monitoring inspection. At the time of the inspection, there were 22 children present with five staff. Children were observed participating in free play activities. There is no medication being stored or administered on site. The sample size was five staff and five children's records.
Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.
Evidence: The records for child #1 and child #4 were each missing addresses for of the two required emergency contact person.
Plan of Correction:
Director will contact parents to obtain addresses and place them in the records.

Description:
Based on review of five staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's record.
Evidence: The record for staff #5 did not contain two references as to character and reputation being checked prior to employment.
Plan of Correction:
Center director will ensure that all references are checked and results are placed in the records.
Standard #:
22VAC40-191-60-C-2
Description:
Based on review of five staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry on file within 30 days of employment.
Evidence: The records for staff #1, staff #3, and staff #4 did not include a completed search of the central registry.
Plan of Correction:
Center director will ensure that all staff have completed searches on file.
