Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios, CPR certification, out of state searches, and accessing the department's website for notices, forms, and additional information. A copy of the new regulations was given to the director during this inspection.
Comments:
An unannounced monitoring inspection was conducted today, July 22, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 12:14pm and concluded at approximately 1:45pm. Today approximately 9 children were in attendance being supervised by 3 staff. Administrative records, children's classrooms, restrooms, outdoor playground, and common areas were inspected. See attached notice for violations found during this inspection.

Based on the inspection of administrative records, the required fingerprint background check documentation was not present to review.
Evidence:
During the inspection of three staff records, it was discovered staff #1 did not have documentation of a completed fingerprint background check. Fingerprint background checks were to be obtained by September 30, 2018 with documentation maintained to review.
Plan of Correction:
Per administrator: We will locate the background check and place it in the staff's record. If we're unable to find it, we will call the OBI office to obtain a copy.
