Based on a review of five records for children, and an interview with the provider, the provider did not obtain documentation of additional immunizations for a child at least once every six months under the age of two years. Evidence: Child 4 will not be two years in age until November 2020 and the most recent immunization report was dated April 10, 2019.
Plan of Correction:
The child just had another doctor visit. Another signed immunization record will be obtained from the parent.

Description:
Based on a review of documented annual training, and an interview with the provider, the provider did not obtain at least sixteen hours of annual training in areas relevant to the job responsibilities. Evidence: There was written information for the completion of ten and one-half hours of training by October 2019. The provider indicated she is working on another on-line training this month and thought the hours were due at the end of 2019.
Plan of Correction:
The provider will make sure she completes at least sixteen hours of training by November 14, 2020. Additional sources for training on-line will be considered.
