Comments:
An unannounced monitoring inspection was conducted on 11/08/2023. There were 30 children, ages 3 years-5 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed arriving to the program, having an afternoon snack, participating in learning activities, and in large motor play outside. A total of 5 children?s records and 3 medications were reviewed; staff records will be reviewed on a later date at the administration office. The inspector arrived to the center at 2:35 pm and departed at 4:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on observation, the center failed to ensure that children under 10 years of age always shall be within sight and sound supervision of staff.
Evidence: 1. Child 2 (4 years old) was observed, with Staff 4?s permission, walking down the hallway and into the classroom alone to get a snack. Child 2 was not within sight and sound supervision of staff while in the classroom. 2. Child 3 (4 years old) was observed, with Staff 4?s permission, walking down the hallway and into the classroom alone to get their lunchbox. Child 3 was not within sight and sound supervision of staff while in the classroom.
Plan of Correction:
Will keep a close watch on the children and review with the staff.
