Comments:
An announced staff record review inspection was conducted on 05/15/2024. A total of 4 staff records were reviewed. The inspector arrived at 9:00 am and departed at 10:55 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and discussion with staff, the center failed to ensure that every employee of a licensed child day center shall undergo a background check in accordance with the Code of Virginia prior to issuance of a license and every five years thereafter.
Evidence: The most recent criminal record check for Staff 1 was dated 01/28/2019; there was not a follow-up criminal record check obtained by 01/28/2024.

Based on record review, the center failed to ensure that staff records contain all of the elements required by the standards.
Evidence: The record for Staff 4 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment; there was only one reference check in the record.
