Comments:
This inspection was conducted by licensing staff both on-site and using an alternate remote protocol, including telephone contacts, documents review, interviews and a tour of the program.

A complaint inspection was initiated on 6/1/21 and concluded on 8/25/21. A complaint was received by the department on 6/1/21 regarding allegations in the area of children's injuries and toileting procedures. The center director was contacted by telephone to conduct the investigation. Interviews with staff were conducted both on-site and by telephone. Documentation related to the allegations was also reviewed. Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on interviews and a review of center documentation, it was determined that the center failed to ensure that parents were notified by the end of the day of any known injuries.

Evidence:
1. The parent of child 1 reported that on the evening of 2/3/21 she was giving her child a bath and observed a "bloody" bite mark on her child's nipple area.
a. Child 1's parent stated that she was not told of any injuries to her child at pick-up from the center that day and was not provided with an injury report.
b. Child 1's parent stated that she emailed the center to ask about the bite mark and was told that child 1 was bit that day by another child in the class and that the injury report was located in the office.
2. The injury report for child 1 documents that the child was injured on 2/3/21. The parent signed the injury report on 2/4/21. There was no documentation that the parent was notified on the same day the injury occurred.
3. Staff 1 confirmed that child 1's parent was not notified of his injury by the end of the day, as required by licensing standards. Staff 1 stated that the injury report for child 1 was completed at the time of the incident but center staff did not provide the report to the child's parent at pick-up.

Plan of Correction: The center responded with the following: Per the licensing standards, the incident report had the required two signatures but we failed to give the report to the parent at pickup that evening. After communicating with the parent via email that evening, we immediately put into place another safety protocol due to COVID-19 to send out all incident reports via our parent communication app. The child's teacher will take a picture of the incident report with the required information and signatures and send it to the parent during the child's day. A paper copy of the report will be kept in the front office for all parents/guardians at pickup. Since this protocol has been put into action, we have not had any issues with the notification of incident reports to our parents.
