Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 11, 2019. The on-site inspection was from 10:05 a.m. to 12:20 p.m. On this date, at the time of arrival, there were ten children in the care of the provider (27 points). There were a total of 13 children enrolled in the program. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The records for five children were reviewed. There was no medication being administered, which is the policy of the provider. The records for the provider, an assistant and one household member were reviewed. The Letter of the Week was H,h and the children had doing a math activity with beans on a tray.
Some items reviewed/observed included: first aid kit, emergency supplies, required posted information, documentation of evacuation and shelter-in-place drills, warm water temperature, temperature of the fridge and freezer, hand washing procedures, infant care, and TV time.

Description: Based on observation, record review and verification from the provider, the provider failed to have a second caregiver on-site when there were more than 16 points worth of children present.

Evidence:
1. There were 10 children present and in the care of one provider which was 27 points based on the ages of the children. The provider had 11 more points than the 16 allowed for each caregiver.
2. The provider confirmed she had more points than she was allowed to have at any one time. She reported that her assistant was driving to work this morning when she became sick and had to go home.

Plan of Correction: The provider will hire an assistant that can come anytime she is needed.
