An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing office on 7/25/19. Interviews were conducted with staff and records were reviewed. There was information gathered during the inspection that resulted in a violation. A plan of correction was obtained by the main office for CVCA Head Start program. Based on interviews and review of policies and procedures the center failed to follow the field trip procedures for this program. Evidence: 1. Staff # 1 & 2 did not follow the agencies field trip policy on 5/23/19.
Plan of Correction:
Situation was discussed with staff; 3 staff members received written letter of reprimand. Policy was updated in procedure manual and reviewed with all staff.
