Comments:
An unannounced non-mandated monitoring inspection was completed on 02/25/2020 from 9:15 A.M. to 11:30 A.M. The inspection was conducted to a response of a self-reported incident that occurred at the center on 01/23/2020. One child's records, three staff records, medication authorization form, and two staff members were interviewed. The exceptions to compliance are noted on the violation notice.

Based on an interview with administration, the center failed to keep a record of over the counter skin products given to children.
Evidence: 1. Diaper cream was administered to Child A on 01/23/2020. 2. Staff A stated she spoke with Staff #3. Staff #3 stated that she had applied diaper ointment to Child A on 01/23/2020. 3. Staff A verified that there was no documentation of diaper ointment being applied.
Plan of Correction:
Applying and documenting diaper ointment was discussed at the staff meeting on 02/24/2020. The director created a form for documentation of the diaper ointment.
