Comments:
Licensing Inspector arrived at 12:15 and departed at 1:30 PM to conduct an unannounced renewal inspection. At the time of the inspection, there were 10 children (ages infant through preschool) in care with three staff. Children were observed doing circle time activities, transitioning and eating lunch. The sample size was four staff and four children's records.
Based on review of four children's records, it was determined that the facility did not ensure that all required information was present in each child's record.
Evidence: The records for child #3 and child #4 did not include addresses for emergency contact persons.
Plan of Correction:
Center management will contact the parents to obtain addresses and place them in the records.

Description:
Based on observation, it was determined that the facility did not ensure disposable diapers were disposed of in a leakproof or plastic-lined storage system that is either foot operated.
Evidence: The lid to the diaper disposal system in the infant area was ajar, not allowing the foot operated mechanism to operate correctly.
Plan of Correction:
Center management will ensure that all disposal cans are foot operated.
