Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews. A self reported allegation concerning care of children was received from the facility. Based on the preponderance of evidence, the allegation is not valid. A violation was issued for injury reports. Based on record review, an injury report is incomplete. Evidence: An injury report for Child #1 was missing "any future action to prevent reoccurrence"
Plan of Correction:
Management will ensure all future injury reports are complete.
