Comments:
An unannounced monitoring inspection was conducted today from 10:50am-11:30am. There were 7 children directly supervised by 2 caregivers. The home, 2 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely with blocks and identifying colors. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on record review, the family day home failed to obtain a sworn statement or disclosure at the time of hire for one staff member. Evidence: Caregiver 2, hired in November 2023, did not have a sworn statement completed until February 19, 2024.

The Sworn disclosure was obtained on 2/19/2024.

Based on record review, the provider failed to review the emergency plan at least annually and update the plan as needed. Evidence: The emergency plan was last reviewed on June 20, 2022.
