Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 1/4/2021 and concluded on 1/11/2021. The provider was contacted by email to initiate the inspection. There were 3 children present, totaling 8 points, with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 household records submitted by the provider to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based upon review of two children's files and provider interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record provider for child 1 indicated that child 1 began attending on 8/17/2020. The record did not include documentation of a physical examination. 2. The provider was unable to provide documentation of a physical examination for child 1.
Plan of Correction:
The provider responded with the following: Documentation of physical examinations will be obtained prior to a child attending or within 30 days after a child has begun attending.
