Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A monitoring inspection was initiated on 07/19/2021 and concluded on 07/27/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 16 months to 4 years, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 caregiver record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the family day home.
Based upon review of two children's records and provider interview, the provider has not ensured that each child's record includes the addresses for two persons to be contacted in an emergency if a parent cannot be reached. Evidence: 1. The record for child 1 did not include the street addresses for the two emergency contact persons listed. 2. The provider confirmed that the parent had not completed the address information on child 1's registration form.
Plan of Correction:
The provider responded with the following: The addresses of the emergency contacts are listed in the siblings file as shown to the inspector. The parent will be required to complete this information in the records for both children.
