Technical Assistance:
Technical Assistance offered today: Monitoring time infants spend in one space, Injury reports.

Comments:
An unannounced renewal inspection was conducted from 10:50am to 1:30pm. There were 27 children present with 10 staff members. Infants were observed getting bottle fed, engaged in floor time activities, and getting diapers changed. The toddlers and preschool children were observed engaged in free play, and playing with play dough. The children were also observed preparing for lunch. 5 children's records and 5 staff records were reviewed. Other documents reviewed included emergency drill records, injury reports and injury prevention plans. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on staff interview, the center did not maintain a complete record for an employee. Evidence: 1. Staff # 1's first day of employment was 8/5/19. 2. Staff # 2 stated that references were not obtained for Staff # 1 before the first day of employment.

Plan of Correction: We will obtain references as soon as possible.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center's written record of children's injuries did not include all required fields. Evidence: The center's written report to record children's injuries was missing the following information: a. date and time when parents were notified, b. staff signatures, and c. method of parent notification.

Plan of Correction: The report was corrected during the inspection.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on staff interview, the center did not obtain fingerprint based criminal history results for an employee before the first day of employment. Evidence: 1. Staff # 1was observed working in the infant classroom. 2. Staff # 2 stated that Staff # 1's first day of employment was 8/5/19. Staff # 2 stated that Staff # 1 has completed the fingerprint process for the center.

Plan of Correction: Staff will make appointment today for fingerprints.
