Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a concern regarding the center?s physical plant, supervision of children, and injury reporting that was received by the licensing department on May 24, 2019. The investigation was initiated on June 5, 2019 and completed on July 18, 2019. On June 19, 2019, two licensing inspectors were on site from approximately 10:10am to 12:30pm. Interviews with staff were completed, observations were made at the facility, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation, therefore the complaint is determined to be valid. See the violation notice for more details.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description: Based on a review of documentation and interview, the center did not ensure a written record was maintained of all children's serious and minor injuries in which entries are made the day of occurrence.

Evidence: 1) During the inspection on 06/19/2019, the licensing inspector interviewed staff members regarding an injury that Child #1 (9-month-old) received at the center during the week of 05/20/2019-05/24/2019. The staff interviewed reported that Child #1 hit her forehead on a bookshelf and it left a ?red mark,? but an incident report was not completed on the day the injury occurred.

2) During interview on 06/19/2019, Staff #1 reported that the center did not have a written record of Child #1's incident report for a minor injury that occurred at the center during the week of 05/20/2019-05/24/2019. Per Staff #1, the incident report was given to Child #1's parent and a copy was not retained at the center. The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
