Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy monitoring inspection was initiated on December 31, 2020 and concluded on March 2, 2021. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the non-compliance with subsidy regulations, and violations were issued.
Based on interviews and documentation review, the vendor did not inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence: 1) On 12/17/2020, Child #1 was injured at the center. The vendor was notified on 12/18/2020 that the child was seen by a physician on 12/17/2020. 2) The vendor submitted the serious injury report to the department on 12/23/2020.
