An unannounced inspection was conducted on 12/01/2023 from approximately 10:10 a.m. ? 4:00 p.m. The inspector reviewed the areas of noncompliance identified in the Intensive Plan of Correction (IPOC). The information gathered during this inspection determined continued noncompliance of the applicable standards. 8VAC20-780-(5)-340-F (Repeat Violation) Based on observation the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence: A child, age 3, was observed by licensing inspector walking down the hallway by himself. He entered a hallway bathroom without staff providing actual sight and sound supervision.
