Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.
A monitoring inspection was initiated on February 18, 2022 and concluded on February 25, 2022. The provider was contacted by telephone/video call and a virtual inspection was conducted. There were 2 children present, ranging in ages from 21 to 22 months, with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of 2 children?s records and 2 caregiver/household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on documentation review, the provider did not have documentation of a obtaining a physical for two enrolled children.
Evidence: 1. Two children records were reviewed. 2. Child #1 and Child #2 did not have documentation of a physical examination. 3. The provider confirmed she did not have documentation of a physical exam for Child #1 and Child #2.
Based on documentation review, the provider did not have documentation of one child obtaining immunizations prior to attending.

Plan of Correction:
I will request a physical exam and immunization report from parents.
