Technical Assistance:
Technical assistance was provided on following up on background checks not received within 30 days. When results are received forward to inspector.
Comments:
A complaint was received in the licensing office regarding staff qualifications and training, background checks, ratios and staff lack of knowledge on food sensitivities. On January 21, 2020 an on-site investigation was conducted. Staff and management were interviewed, 13 staff records and food policies were reviewed. At the time of this investigation, there was one valid finding regarding the nature of the complaint. See violation notice for additional information. Based on review of 13 staff records, two have lived out of the state of Virginia within the past five years and do not have documentation of eligible out of state background checks from the states they had lived in. Evidence: Staff #1 (date of hire: 7/11/19) lived outside the state of Virginia within the past five years. Staff #1 does not have documentation of an eligible back ground check for the state they had lived in. Staff #2 (date of hire: 7/1/19) lived in two states outside the state of Virginia within the past five years. Staff #2 does not have documentation of a eligible background check from the states they had lived in.
Plan of Correction:
We follow up with the states again today. Results will be sent to inspector when they are received.
