Comments:
An unannounced monitoring inspection was conducted on 8/29/2022 from11:00am-1:15pm. There were 140 children directly supervised by 35 staff. The physical plant, 5 staff records, 5 children?s records, 3 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Diaper changing and handwashing were observed. Children were observed participating in group play, coloring, playing outside, and eating lunch. There was an assortment of age-appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice. Standard #: 8VAC20-780-160-A
Description: Based on record review and staff interview, the center did not ensure that a tuberculosis screening was completed within the last 30 calendar days of the date of employment .

Evidence:
1. Date of employment for Staff # 2 was 5/23/22 and the tuberculosis screening on file was dated 06/07/2022.

2. Date of employment for Staff #3 was 8/22/22 and the tuberculosis screening on file was dated 12/2/2021.

Plan of Correction: We will ensure that all new hires obtain a TB test prior to their start date.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the areas and equipment of the center were not maintained in a safe condition.

Evidence: In the Athens classroom, a small laminator was located on a table, plugged in, and accessible to children.

Plan of Correction: We will ensure that all laminators remain in our teacher resource center.
