Comments:
An unannounced monitoring inspection was conducted on 2/22/2024. There were 6 children in care, ages 8 months ? 4 years, totaling 16 points, 2 caregivers and 1 household member present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 3 children?s records, 2 caregiver records and 2 household members records were reviewed. The children were observed during naptime. The inspector arrived for the inspection at 11:50am and departed at 1:00pm.
Discussion on emergency preparedness plan.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of two caregiver records, the provider failed to ensure that all background checks were complete. Evidence: 1. Caregiver 1 most recent criminal background was dated 1/25/2019 and on the day of the inspection did not have the required 5 year repeat check documented. 2. Caregiver 2 most recent criminal background was dated 1/25/2019 and on the day of the inspection did not have the required 5 year repeat check documented.

Provider will submit for 5 year repeat record checks.

Based on observation, the provider failed to ensure that the diapering surface shall be nonabsorbent and washable.
Evidence: The changing pad in the hallway bathroom had a tear approximately 1/4 inch long on the left side and the padding was exposed.
