Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A renewal inspection was initiated on August 26, 2021 and concluded on (date). The director was contacted by telephone and a virtual inspection was conducted. There were 31 children present, ranging in ages from 2 years to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the child did not have documentation of a physical exam within one month of attendance.
Evidence: Child 1 (enrolled 3/2021) did not have a physical exam on file.
Plan of Correction:
I will talk to the parents and ask them for the documentation of the physical.
