Comments:
An unannounced monitoring inspection was conducted from 9:30 am to 10:20 am. At the time of entrance, 7 children were in care with the provider and assistant provider. The record sample size consisted of 5 children's records and the provider's record and the assistant's record. The children in care were observed engaging with the provider, participating in educational activities and free play. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on observation and interview it was determined that the provider did not ensure that there was a carbon monoxide detector in the home. Evidence: 1. There was no carbon monoxide detector in the home. 2. The provider confirmed that there was not a carbon monoxide detector present during the inspection.

Provider will install a carbon monoxide detector.

Description:
Based on record review and interview it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. The record for child #4 did not contain documentation of a physical exam. 2. Provider confirmed this documentation was not available during this inspection.
