22VAC40-665 Subsidy Program Vendor Requirements for Family Day Homes
Comments:
A Subsidy Health and Safety inspection was conducted on 10/3/19 to review supplemental health and safety requirements. Upon a arrival there was no children in care. A licensing inspection was also conducted the date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirement that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Inspection began: 9:50 am ended 12:10 pm.
Based on review of records the vendor failed to have a written allergy plan for children in care. Evidence: 1. Child # 4 & 5 have an allergy to a food but, there are no written allergy plans on file.
Plan of Correction:
The vendor stated that she had the parent fill out this form and it was not completed correctly and she sent it back to have it corrected. The vendor will remind the parent to return this form.
